Decontamination & Safety Disclaimer for Incoming Equipment

By sending equipment, devices, or units for service, repair, or evaluation, the sender certifies that all items have been properly decontaminated and are free from hazardous materials, including but not limited to biological agents, chemical residues, radioactive substances, and other potentially harmful contaminants.

The sender agrees to:

  • Comply with all applicable local, state, and federal regulations regarding the decontamination of equipment.
  • Provide completed decontamination form either electronically prior to shipment or attached to the OUTSIDE of the package.

The receiving company reserves the right to:

  • Refuse, or return any equipment suspected of contamination at the sender’s expense.
  • Delay service until satisfactory decontamination certification is provided.

The sender assumes full responsibility and liability for any damages, injuries, or regulatory violations arising from failure to adequately decontaminate units prior to shipment.
